Core concepts
Concept 1
Professional accountability means being able to justify decisions, act within competence and raise concerns when safety is threatened.
Exam cue: Protect the person first, maximise participation, then identify lawful authority and the correct escalation route.
Concept 2
Decision-specific capacity, supported decision-making, consent, confidentiality, safeguarding and applicable mental health law must be applied without assuming incapacity from diagnosis.
Exam cue: Choose the action that is lawful, proportionate, documented and transparent.
Risk pitfalls and guardrails
Following a local instruction that conflicts with the Code or law.
Guardrail: Do not select a familiar task if it ignores deterioration, consent, medicines risk, scope, communication or follow-up.
Waiting for proof while an immediate safety risk remains.
Guardrail: Choose the action that addresses the most serious current risk before routine assessment, referral or documentation.

Taking responsibility for decisions, actions, omissions, reasonable adjustments and appropriate delegation.
What does candour require?

What is the capacity starting point?
Do not assume incapacity because a person has a learning disability; support the specific decision and follow applicable law.
What makes an accountable record?
Contemporaneous facts, assessment, decision, action, escalation and outcome.
