Shared decision-making presents reasonable options, benefits, harms and uncertainty in a way the child and family can use, within lawful authority.

Structured handover and accurate records protect continuity when urgency or complexity is high.
Exam cue: Acknowledge the child's and family's emotions before adding more technical information.
Risk pitfalls and guardrails
Overloading the person immediately after distressing information.
Guardrail: Choose the action that addresses the most serious current risk before routine assessment, referral or documentation.
Giving a vague handover without assessment, recommendation or urgency.
Guardrail: Do not select a familiar task if it ignores deterioration, consent, medicines risk, scope, communication or follow-up.
